Thank you for being part of the Community Data Program. In order to continue to serve our members through access to data, delivery of program resources and tools, and improving our network, we ask that you complete the following survey questions based on your experience with the program. Your answers will be used by the CDP Team to improve the program and website, create community snapshots and other local projects and products so that your work can be shared through the network, and complete our annual report.

The survey should take less than 5 minutes of your time.

The results of the survey will be anonymized. We will be rolling up results for reporting purposes. Should we want to share any quotes or comments attributable to a named individual, we will seek permission from the respondent before doing so.
